• We will make preparations to protect against data loss due to natural and manmade disasters, including regularly backing up all information
• We will monitor our data on a regular basis, and take necessary measures to correct the situation should your personal information be compromised in any way
  
• We will ensure that all employees and organizations associated with our business are fully aware of our privacy policy and fully trained in executing it

• In order to secure the privacy of your personal information, our management system will be monitored internally and updated regularly

1. All requests should be processed as follows

Please fill out the “Request for Release of Personal Information” form in its entirety, enclose the required documents (see below), and mail to us at the address above. We ask that you mark “Request for Release of Personal Information” on the envelope, and use a trackable mailing system for your records.

2. Identification documents

Please enclose one of the following for identification purposes: copy of a valid driver’s license, an original “Certificate of Residency” (domicile need not be indicated), or a copy of your company/organization/country issued health insurance card.

3. Request for disclosure by a representative

If a representative is making the request on your behalf, please enclose the following documents:

• Identification documents for the representative – copy of a valid driver’s license, an original “Certificate of Residency” (domicile need not be indicated), or a copy of the representative’s company/organization/country issued health insurance card.
• Authorization for representation (ininjo) with your seal and a certification of your seal (inkan shoumeisho). If your representative is a legal guardian, please also enclose a family registry (or extract) or a certificate of residency with information of your family relationship.

4. Fees for disclosure of information

A processing fee of JPY1000 will be required for each request. Please enclose a money order (yubin teigaku kogawase) from the post office for JPY1000. Please be advised that we will not be able to process your request if the proper processing fee is not enclosed.

5. How we will respond to your request for disclosure

We will respond in writing to the addressee.
